INSTRUCTIONS FOR SUMMER CAMP MAIL REGISTRATION

Thank you for your interest in our Summer Gymnastics Programs.
In order to enroll your child in the program, please:

1. Print and fill out the Registration Form or the Mini Camp Registration Form and indicate your class preference.

2. Print, Read and sign the Membership Agreement. Print and keep one copy of Membership Agreement for your future reference.

3. Use the Payment Sheet to calculate the amount due.

4. Return both forms (stapled together) to our office with proper payment.
